Am I eligible for home health?
Medicare home health has four basic requirements. We help patients and families confirm eligibility during the referral call.
- You are under the care of a physician who signs a plan of care.
- You have a skilled need — nursing or therapy — that requires a licensed clinician.
- You are considered homebound (leaving home requires considerable effort).
- You have Medicare, Medi-Cal, or another participating insurance plan.
