This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

Our commitment

Supreme Home Health Agency, Inc. is required by law to maintain the privacy of your Protected Health Information (PHI), to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How we may use and disclose your PHI

  • Treatment. We use your PHI to provide, coordinate, or manage your care with physicians, therapists, and other providers.
  • Payment. We use your PHI to bill and collect payment from Medicare, Medi-Cal, insurance plans, or you.
  • Health care operations. We use your PHI for activities such as quality assessment, staff training, licensing, and business planning.
  • As required by law. We may disclose PHI when required by federal, state, or local law.
  • Public health & safety. Reporting communicable diseases, suspected abuse or neglect, and serious threats to health or safety.

Your rights

  • Inspect and receive a copy of your PHI.
  • Request an amendment to your PHI.
  • Receive an accounting of certain disclosures we have made.
  • Request restrictions on certain uses and disclosures. We are not required to agree except in limited circumstances.
  • Request confidential communications at an alternative address or number.
  • Receive a paper copy of this notice on request.
  • Be notified in the event of a breach of your unsecured PHI.

How to file a complaint

If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.

Changes to this notice

We reserve the right to change this notice and to make the revised notice effective for PHI we already have as well as any information we receive in the future. The current notice is available at our office and on this website.
