What to include
To start care quickly, please include as much of the following as possible. We will coordinate any missing items directly with the ordering physician.
- Patient demographics and current address
- Insurance information (Medicare, Medi-Cal, or commercial plan)
- Referring physician name, NPI, and contact
- Signed face-to-face encounter documentation
- Verbal or written orders for home health services
- Recent history & physical, discharge summary, or progress notes
- Medication list and known allergies
